Vitae Caring Foundation - Official Contest Rules NO PURCHASE OR INTERNET ACCESS NECESSARY – VOID WHERE PROHIBITED 1. Eligibility. The Contest is offered only in the United States to legal United States residents. Employees and directors of Vitae Caring Foundation (“The Caring Foundation”), and the immediate family (spouse, mother, father, sister, brother, daughter or son, regardless of where they live) or members of their same households (whether related or not) of such employees and directors are not eligible. By entering, you agree to these Official Contest Rules and the decisions of Vitae Caring Foundation, the Contest judging organization, which are final and binding in all respects. 2. Contest Period. The Contest period begins on the first calendar day of each month at 7:00 a.m. Eastern Standard Time (EST), and ends on the last calendar day of each month at 11:59:59 p.m. CST ("Contest Period"). Throughout the Contest Period there will be one prize drawing. 3. Entry. IF SUCH MINOR DOES NOT RETURN A COMPLETED PARENT PERMISSION FORM WITHIN THIS PERIOD, HE/SHE WILL FORFEIT HIS/HER RIGHT TO BE DECLARED THE SWEEPSTAKES WINNER AND AN ALTERNATE WINNER WILL BE SELECTED. 4. Drawing. The Contest winner will be selected by random drawing from all eligible entries received for that drawing. One drawing will take place from all eligible entries received. To be eligible for a drawing, entries must be received by 11:59:59 p.m. on the last day of the Contest Period. Winners of any drawing will be notified by email, telephone, overnight carrier, or regular mail within approximately 10 days of the drawing. Drawings will be conducted under the supervision of Vitae Caring Foundation/Kathy Guerin Creative, Inc. Prize/Odds. In accordance with these Official Contest Rules, the following prizes will be awarded: $100 Amazon.com Gift Certificate. Limit one prize per person or e-mail address (regardless of whether more than one person uses the same e-mail address) per calendar year. The total number of entries will determine odds of winning at time of drawing. No assignment, transfer or substitution of prize is permitted except by Vitae Caring Foundation in the event of prize unavailability. Taxes, if any, are the sole responsibility of winner. General Conditions. Winners must execute and return an affidavit of eligibility and where lawful, a publicity release within 5 days of notification. Failure to return documents timely, or if prize notification or prize is returned as non-deliverable, may result in disqualification with an alternate winner selected. Where permitted by law, winner agrees to grant to Vitae Caring Foundation, the right to print, publish, broadcast and use, worldwide in any media now known or hereafter developed, the winner's name, portrait, picture, voice, likeness and biographical information as news or information and for advertising and promotional purposes without additional consideration; and further without such additional compensation, appear for, or provide biographical information for use in, any presentation or other activity, as may be determined from time to time by Vitae Caring Foundation. Failure to make such appearances or grant such rights may result in disqualification with an alternate winner or potential winner selected. Further, by accepting a prize, each winner agrees to release and hold harmless Vitae Caring Foundation and Kathy Guerin Creative, Inc. from any and all liability for any injuries, loss or damage of any kind to person, including death, and property, arising in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of a prize, participation in any Contest related activity, or participation in the Sweepstakes. 5. Vitae Caring Foundation reserves the right, at its sole discretion, to disqualify any individual it finds, in its sole discretion, to be tampering with the entry process or the operation of the Contest or website; to be in violation of the website Terms of Use, to be acting in violation of the Official Contest Rules; or to be acting in a disruptive manner, or with intent to annoy, abuse, threaten or harass any other person. Any use of robotic, automatic, macro, programmed or like entry methods will void all such entries by such methods. In the event of a dispute as to entries submitted by multiple users having the same email account, the authorized subscriber of the email account used to enter the Contest at the actual time of entry will be deemed to be the participant and must comply with these rules. Authorized account subscriber is deemed to be the natural person who is assigned an email address by an Internet access provider, online service provider or other organization which is responsible for assigning email addresses or the domain associated with the submitted email address. Vitae Caring Foundation will prosecute any fraudulent activities to the full extent of the law. 6. Limitations of Liability. Vitae Caring Foundation is not responsible for any incorrect or inaccurate information, whether caused by website users, or tampering or hacking, or by any of the equipment or programming associated with or utilized in the Content and assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to the website. Vitae Caring Foundation is not responsible for injury or damage to participants' or to any other person's computer related to or resulting from participating in the contest or downloading materials from or use of the website.
